review your dental and medical history, take X-rays of your teeth and mouth, and meet with you
to discuss your X-rays and develop a personalized treatment plan. We’ll also address any
questions you may have

What to Bring to Your First Appointment                                           

To ensure a smooth and efficient visit, please remember to bring the following:

• Completed patient information form (please create a link to the patient forms section)
• Any x-rays taken by a previous dentist (if applicable)
• A list of medications you are currently taking
• Your insurance card

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
